An Affidavit from a First Class Judicial Magistrate (Download).
Fill up of downloaded prescribed Form.
Fill up of downloaded prescribed Form.
Payment of Fees for Surname change.
Now, after uploading the downloaded form and an affidavit from 1st Class Judicial Magistrate the Applicant has to submit Original Registration Certificate alongwith the Money receipt paid for the purpose within 7 working days to the office of the Council.

Any Daily News Paper Publication/ Advertisement on the matter.

Payment of Fees for Surname change (Payment Link).
Now, after uploading the above documents the Applicant has to submit Original Registration Certificate along with the Money receipt paid for the purpose within 7 working days to the office of the Council.
