Procedure to follow if there is any change in Registered Name:-
A Registered Pharmacist who has changed his/her name or a part of his/her name, he/she should submit an application in prescribed form along with documentation of change or alteration in any part of the name and fees of Rs. 650/- (Rupees Six hundred fifty only).

Form for Change of Name.
Marriage certificate (In the case of married female candidate) or A copy of gazette notification (in all other cases).
Any one address proof in new name.
