New Registration for Category A Pharmacists

Only the candidates residing in the state of West Bengal are eligible for registration with WBPC. Candidates are advised to go through the following sections carefully before applying for registration. Incomplete application forms would not be considered for registration. In addition, once the application is submitted you would not be allowed to edit, so enter the details carefully.

Profile

A person who has attained the age of eighteen years and successfully completed a Diploma, Degree or Pharm D Course in Pharmacy from an Institution approved by the Pharmacy Council of India during completion of degree/diploma will be entitled to have his/her name entered as a Pharmacist on the register of the Council and is subject to he/she resides or carries on the business or profession of pharmacy in the State of West Bengal. Candidate will have to come personally to pharmacy council with all original documents as mentioned below alongwith Xeroxed copies at pharmacy council office for registration.

A. Documents to be uploaded For D.Pharma

LIST OF DOCUMENTS (TO BE ATTESTED) FOR D.Pharma:

 All marksheets of diploma in pharmacy.

 Practical training certificate of 500 hours (In original).

  Age proof certificate (Class X Admit Card or certificate).

 Voter I.D Card/ Aadhar Card/Passport

 Character certificates (In original).

 10+2 Marksheet (In Original)

 Three Recent Passport Size Photographs (5 X 4 Cms).

 An affidavit (as per specimen copy enclosed in the case of applicant who has passed Diploma / Degree in Pharmacy examination from an institution West Bengal and other than the state of West Bengal alongwith the undertaking mentioning the authenticity of all documents submitted during New Registration.

 Proof of Employment of the candidate:((a) Employer's Certificate mentioning period of service, designation, head quarters, etc. from relevant Firm/Company/Institution mentioning Drugs License No’s, if any ; or (b) In case of self employed persons, A self declaration on the letter pad of the relevant Firm/Company etc. mentioning Drug License Nos. if any ;

 Undertaking and affidavit (as per specimen in the case of applicant who has been registered as pharmacist in other state Council.

 All Marksheets of B.Pharm.

 Industrial Training Certificate.

 Age Proof Certificate (Class X Admit Card or Certificate).

 Voter I.D Card / Aadhar Card / Passport.

 Character Certificates (In Original).

 10+2 Marksheet (In Original).

 Provisional Certificate.

 Three Passport Size Photographs (5 x 4 cms).

 An affidavit (as per specimen copy enclosed in the case of applicant who has passed Diploma / Degree in Pharmacy examination from an institution West Bengal and other than the state of West Bengal alongwith the undertaking mentioning the authenticity of all documents submitted during New Registration.
